SCHEDULING YOUR CHILD'S BAPTISM
To schedule your child's baptism, please follow these steps:
- Review the documents below: Download and review the Baptismal Guidlines using the following links:
* Baptismal Guidelines (English) * Requisitos Baptismales (Español)
2. Gather the Required Documents: Collect all the necessary documents as specified in the guidelines.
3. Visit the Parish Office: Once you have reviewed the guidelines, completed the Baptismal preparation class and gathered all the required documents, visit the parish office to register your child for baptism.

Printable Forms for Baptism Registration
When completing this form please use legal names or names as they
appear onthe child's birth certificate. Required signatures must be done in
the presence of office staff.
- Baptismal PreparationCompletion Certificate
After completing the online class, a certificate will be available for
printing from Ascension Press.
This certificate is required to register your child for baptism.
This form must be completed and signed by the godparents' church.
