The Archdiocese of Miami provides specific guidelines for the recording of sacraments and the issuing of sacramental certificates. At St. Mark, we follow these guidelines to ensure that every record is accurate, confidential, and properly preserved.
If you or your child received a sacrament at St. Mark, such as Baptism, First Holy Communion, Confirmation, or Marriage, you may request an official sacramental certificate. These certificates serve as official documentation of the sacraments you have received and are often required when preparing for another sacrament, such as Holy Matrimony or Holy Orders.
Certificate Request Guidelines
- The person who received the sacrament may request their own certificate.
- For a minor child, the request may be made by a parent or legal guardian.
- To protect the privacy of our parishioners, certificates cannot be issued to third parties.
- Requests may be submitted in person (with a government-issued photo ID) or by completing the form below.
- Sacramental certificates are not issued for genealogical research.

Kindly allow 48 hours for processing once your request and documentation are received. You will be contacted when your certificate is ready.
